You will need complete the Prospective Faculty Information Sheet electronically, then email it complete to your faculty recruiter.

Here are the instructions on how to submit this form by email:

• Download the Prospective Faculty Information Sheet to your computer and complete it using MS Word. 

• Save the file on your computer where you can find it and close it.

• Open a new email message by clicking on the email for your faculty recruiter listed below.

• It is a good idea to cc: yourself, in case there is any problem with the transmission.

• Attach the completed, saved Prospective Faculty Information Sheet file from your computer.

• Send the email.
